Lost the Curve in Your Neck? What the Research Says About Forward Head Posture

Short answer: your neck is supposed to have a gentle forward curve. When it straightens or reverses, usually from years of looking down at screens, the head drifts forward, load on the neck multiplies, and neck pain, headaches and fatigue follow. A 2022 review of case reports on structural correction reported an average 14° improvement in the cervical curve and an average 80% reduction in pain over roughly 16 weeks of care, with improvements maintained at follow-up.
Your head is heavier than you think
Your head weighs about 4.5 kilos and balances delicately on the small bones of the upper neck, the same bones that protect your brainstem. In a neutral position that is manageable. But the further forward the head goes, the more it acts like a weight held at arm’s length: the muscles of the neck and upper back have to work constantly just to hold it up. That is not only more pain and more headaches; it is energy wasted all day, every day. What could you do with more energy and less pain?
What “loss of cervical lordosis” means
Seen from the side, a healthy neck curves gently forward. That curve absorbs shock and spreads load. When it is lost, the neck becomes straight or even reverses (cervical kyphosis). We see this constantly in people who work at a desk or on a phone, after whiplash, and in teenagers who have grown up with screens. The signs are familiar: neck pain, tension headaches, stiffness in the upper back, tingling into the arms, and a head that sits visibly in front of the shoulders in photos.
There is a neurological side too. The upper neck is the densest area of movement sensors in the body. When those joints lose motion, the information reaching the brain changes, and the brain’s picture of the body changes with it. Research from Dr. Heidi Haavik’s group has shown that adjusting these areas changes the way the brain processes sensory input.
What the research reported
A 2022 selective review by Oakley and Harrison of case reports using Chiropractic BioPhysics (CBP) structural rehabilitation reported, on average:
- A 14° improvement in the cervical curve
- An 80% reduction in pain
- A 12 mm reduction in forward head posture
- Results over roughly 40 visits in about 16 weeks
- Improvements maintained at long-term follow-up
Case reports are not randomised trials, and we say so. But the direction is consistent with what we measure in the studio: this is not temporary relief, it is structural change that shows up on posture photos and scans.

Frequently asked questions
Can a lost cervical curve be restored? The research above and our own progress photos say it can improve meaningfully, particularly with consistent care over several months. How much depends on how long the change has been there and on what you do between visits.
Is it just posture? No. Posture is the visible part. Underneath is how the joints move and how the nervous system is processing that movement, which is why we scan and re-scan.
Do you take X-rays? Not routinely. If imaging is clinically necessary we refer you, and if you already have recent images we use them.
